Additionally, players who purchased tickets in person and won $600 to $49,999 may claim their prizes by mail. Complete the form on the back of the original ticket, sign it, include a copy of two forms of photo ID, and send it all to the following address.
The CT Lottery is not responsible for lost or stolen mail, so winners should make a copy of both sides of the original ticket and use a form of mail that includes proof it was sent and received.
